Notice: Explanation of additional urgent care fees
In our clinic we use two specialty codes and want to make you aware of these in advance. These codes are for Urgent Care Service and Extended Hours of Service. The cost of these services are up to $50 for each.
· The Extended Hours of Service fee is added for service provided after 5p.m. daily, weekends and holidays to help compensate for our higher operating costs during these times.
· The Urgent Care Service fee is added to all visits. These fees help support our organization and help meet the costs of operating 7 days a week with more extensive care services such as in-house laboratory, X-ray services, EKG’s, etc.
These fees are added to the baseline charges for your visit. Most insurance companies recognize these billable charges and will provide partial or full reimbursement.
In the event your insurance company does not cover these additional fees, you may receive a billing statement and you will be responsible for any residual balance.

- We accept payment in the form of Visa, MasterCard, Checks & Cash.
- Co-Payments or deductables are due at the time of service.
- If you are not covered by these health insurance plans, full payment for your visit is required at the time of the service.
